Attention Student-Athletes!

Attention Student-Athletes.  The Lebanon School District will be offering Free Sports physicals for any current 6th-11th grader interested in participating in a school-sponsored sport during the 2021-2022 school year.   Please be sure that you read and understand the agreement statements when scheduling for a Free Sports Physical.  Failure to show up for a scheduled appointment time, and not notifying the athletic office 24 hrs in advance of your scheduled appointment time, will result in a $20 fee added to your student account.  If you need to cancel your appointment please contact the athletic office at (717) 270-6722. Physical Packet 2021-2022 Please be sure to read the instructions in its entirety.  You must complete sections 1-5 of the physical packet and turn it into the athletic office prior to May 28 by 3 PM, NO EXCEPTIONS!  Physical packets will be available to pick up at the end of the week, hopefully, Friday, in the athletic office at the high school and in the main office of the middle school. If you need to download and print a PIAA 2021-2022 Physical Packet.  Please click on the link below or Physical Packets may be picked up in the Athletic Office at the High School or in the Main Office at the Middle School.

Prior to turning in your 2021-2022 PIAA Sports Physical Packet, you must complete the online Physical Information form.   All the information is taken directly from Section 1 of the PIAA Physical Packet. Section 1 of the Physical Packet and the information entered into this form must match.

Athletic Trainers NOT Trainers

Athletic Trainers (ATs) are health care professionals who collaborate with physicians to provide preventative services, emergency care, clinical diagnosis, therapeutic intervention, and rehabilitation of injuries and medical conditions. Certified athletic trainers must earn a degree from an accredited athletic training curriculum. Accredited programs include formal instruction in areas such as injury/illness prevention, first aid, and emergency care, assessment of injury/illness, human anatomy and physiology, therapeutic modalities, and nutrition.
